If you require any vaccinations relating to foreign travel you will need to complete a 'Pre Travel Risk Assessment Form'. These are available to collect at the reception desk or by clicking on the link below.
Please return the completed 'Pre Travel Risk Assessment Form' to reception and our Practice Nurse will contact you by telephone within 10 working days to discuss your vaccination requirements or to sign post you to the nearest Travel Vaccination Clinic. This will only apply if the travel vaccinations are too complex to be carried out in surgery.
Dr Kidd & Dr Cosgrove reserve the right to refuse travel vaccinations if it is felt that the planned travel is too complex. This is a non-NHS service.

Please allow plenty of time before you travel, at least 6-8 weeks to organise vaccinations. Some travel vaccines have to be ordered as they are not a stock vaccine. Some vaccines are only available on a private prescription, this is because not all travel vaccinations are included in the services provided by the NHS.
